GOLF OUTING SIGN-UPS:
In order to help ensure a smooth outing, please ensure if you are signing up an entire team that you have names, cell phone and email for all players in your team.  After your background information is entered in the gray boxes, please select the 'August 8th Golf Outing' box and accept the terms and conditions.  Once you successfully submit the form, you will be directed to 'Success Page'.  If you don't get directed to that page after hitting submit, you missed a required field (scroll up and find your error and then re-submit). Ensure all boxes marked with an '*' are completed as they are required.

The Phantoms will return to the links for an annual golf outing on August 8th at Three Fires Golf Club in West Olive, Michigan.  We are aiming for 100-people to attend this event and this event is open to everyone who signs up.  Cost will be $300 for a team or $75 per person.  (If signing up as an individual, you will randomly be placed with another group to fill the team).  We are working on all the special effects for a golf outing including prizes, specialty holes and other golf related effects.  Additional details to follow.

Terms & Conditions: By agreeing to the terms & conditions, you understand that payment is required by the posted deadline.  Failure to submit payment, could result in you or your team being removed from this event. Additionally, in the event that the player/team cancels after the payment deadline and submitting payment, the Phantoms and venue can not guarantee a full or partial refund.  Attempts will be made to have additional players fill the team or player's spot, but in the nature of the sports & entertainment business, payment to the venues on behalf of the Phantoms is required in advance of the golf outing.  Cancelation terms of the event based on the weather will be handled by golf course. The Phantoms, including organizer Ryker Barber and Kevin Wootton, can not be expected to refund player or team costs in the event a player or team cancels from this event after the payment deadline.
